Community Exploration welcomes referrals from case managers, service coordinators, support brokers, and other professionals serving adults with intellectual and developmental disabilities.
Our program provides engaging, community-based experiences that promote independence, social connections, skill development, and meaningful participation in everyday community life.
Our Services
Participants enjoy a variety of structured activities, including:
Arts, recreation, and health and wellness
Community outings, business tours, and educational experiences
Life skills development and independent living education
Volunteering and community involvement
Social activities and relationship building
Our program typically operates with a 3:1 participant-to-staff ratio, allowing for personalized attention and meaningful engagement.
Eligibility & Individual Support
Community Exploration is designed for adults with intellectual and developmental disabilities who can benefit from supported community participation.
We review each prospective participant's interests, abilities, accessibility requirements, and individual support needs to determine appropriate accommodations and program suitability.
Funding Options
We welcome private-pay participants and are working to establish service agreements with Wisconsin managed care organizations (MCOs) and IRIS.
Participation through Family Care or IRIS is subject to applicable authorizations, funding requirements, and provider arrangements.
Making a Referral
Our referral process is simple:
Contact Us: Share basic information about the prospective participant and their interest in Community Exploration.
Discuss Individual Needs: We'll review goals, support requirements, transportation considerations, and funding options.
Coordinate Enrollment: We'll work with the participant, family or guardian, and care team to complete enrollment and arrange participation.
Explore the Calendar: Participants can select full program days featuring activities that match their interests and goals.
